HOW TO APPLY

Please complete this printable registration form (PDF) and mail or fax to the School with full payment or a non-refundable $100.00 registration fee. Cheques can be made payable to The School of Toronto Dance Theatre. Please do not forget to include your dance résumé.

All fees must be paid in advance of the course.

Registration Checklist

• Completed registration form
• Full payment or $100.00 non-refundable registration fee
• A recent photo, in dance clothes

Detailed dance résumé (maximum two pages) which includes:

• where you have studied
• what style, including any prior Graham training
• how long (years) and how often (times per week)
• with whom
• any performance experience
• contact information for three dance references
